In order to enroll in AOI HealthChoice, an AOI Member Application must be completed. Please complete this form in full and mail to AOI with applicable membership dues for processing to 1149 Court St. NE, Salem, OR 97301.

For new employers enrolling in the AOI HealthChoice program.

To be completed for the employer's annual renewal process only.

To be completed by all new or existing eligible employees for plan enrollment or when changes are necessary.
